Clinical questionnaires

Measure progress the same way every time

Send PHQ-9, PHQ-8, GAD-7 or PEG by a personal link. The patient answers on their phone, the score is calculated as published, and the results are charted in their file.

A measurement that repeats the same way

"Feeling a bit better" is hard to compare with "no change". A validated questionnaire, answered the same way each time, turns the course of treatment into numbers you can show the patient.

What is included

From sending to the chart

Validated questionnaires

PHQ-9 for depression symptoms, GAD-7 for anxiety and PEG for pain. No questions to type and no scoring to set up.

Score and severity

The score is calculated as published the moment the patient finishes, with a severity band for PHQ-9, PHQ-8 and GAD-7. PEG is the average of its three items, from 0 to 10.

A personal one-time link

Emailed from your dashboard, or copied and shared your own way. The patient answers on a phone without an account.

A chart for each questionnaire

Results over time, charted separately for each questionnaire once there are two.

In the clinic too

Hand the patient a device and they answer then and there, through a link valid for two hours.

Logged

Every questionnaire sent and every answer received is recorded in the access log, and so is each time you open the results.

How it works

Four steps, no paper

1

Choose and send

From the patient's file choose PHQ-9, PHQ-8, GAD-7, PEG or several at once. PHQ-9 is offered once you confirm how your practice responds to a self-harm answer; PHQ-8 leaves that question out. Email the link, or copy it and share it any way you like.

2

The patient answers on their phone

The link is personal and works once. No account or password, and it expires after 14 days.

3

The score is calculated and saved

The total, the severity band for PHQ-9, PHQ-8 and GAD-7, and every answer are stored, encrypted, in the patient's file.

4

Follow the trend

Each questionnaire gets its own chart, so the change across treatment is visible at a glance.

In the clinic or at home

Three ways to get it to the patient

Send it before the visit, share the link your own way, or have the patient answer on your device in the treatment room.

  • Emailed from your dashboard, or copied and shared any way you like, valid 14 days
  • Answered on your device in the clinic, valid two hours
  • Several questionnaires sent together, and a link not yet used can be cancelled

Risk answers

Question 9 on the PHQ-9 gets its own handling

That question asks about thoughts of self-harm. A positive answer has to reach you, and it must not get lost among other results.

The moment it is answered

An alert appears at the top of the patient's file and across your dashboard, and you are emailed, with no health details in the email.

What the patient sees

The 988 Suicide & Crisis Lifeline, shown as soon as they answer.

Until you review it

The alert stays until you mark it reviewed. Who reviewed it, and when, is recorded; the answers stay in the file.

The alert draws your attention. Assessing the risk and acting on it remain your responsibility.

Frequently asked questions

Which questionnaires are included?

PHQ-9 (depression symptoms), GAD-7 (anxiety symptoms) and PEG (pain intensity, and how pain interferes with enjoyment of life and general activity). Each is worded and scored as published.

Can I write my own questionnaire?

Yes. Alongside the validated questionnaires you can write your own: rated questions that add up to a score, choices and open answers. Editing one saves a new version, so every answer stays next to the questions that were asked. Your own questionnaires are not validated instruments, so they have no severity bands.

Does the patient need an account or an app?

No. The patient opens a personal link and answers. The link works once and expires: after 14 days when it is emailed or shared, after two hours when it is answered in the clinic.

How do I send a questionnaire?

From the patient's file. Email it from your dashboard, copy the link and send it any way you like, or hand the patient a device in the clinic.

What happens if a patient indicates thoughts of self-harm?

If PHQ-9 question 9 is answered above zero, an alert appears at the top of the patient's file and across your dashboard, and stays until you mark it reviewed. You are emailed, with no health details in the message, and the patient is shown the 988 Suicide & Crisis Lifeline. The alert draws your attention; assessing the risk and acting on it remain your responsibility.

Where are the results kept?

In the patient's file, encrypted. Sending, answering and your viewing of the results are all recorded in the access log.

Which plans include it?

Every plan: Starter, Growth and Pro.
